Ring & Messmer grading of anaphylaxis severity

Classifies an acute hypersensitivity reaction into severity grade I–IV by the most severe organ involvement.

Formula / logic

Grade I: skin/mucosa only (flush, urticaria, angioedema). Grade II: measurable but non-life-threatening cardiovascular (hypotension, tachycardia), respiratory (dyspnea) or GI symptoms. Grade III: life-threatening, shock, severe bronchospasm, laryngeal edema, loss of consciousness. Grade IV: cardiac and/or respiratory arrest.

Interpretation

Widely used (with adaptations) to grade peri-procedural and immunotherapy reactions. Grade ≥II represents anaphylaxis.
