RegiSCAR DRESS validation score

Scores the likelihood that a presentation is DRESS (drug reaction with eosinophilia and systemic symptoms): excluded / possible / probable / definite.

Evidence tier: Validated instrument.

Formula / logic

Sum of 10 items (fever, lymphadenopathy, atypical lymphocytes, eosinophilia, skin extent, rash morphology, biopsy, organ involvement, resolution ≥15 d, exclusion of other causes); range −4 to +9. Interpretation: <2 excluded; 2-3 possible; 4-5 probable; ≥6 definite.

Interpretation

The RegiSCAR score standardizes DRESS case ascertainment. Negative points (no fever, no DRESS-type rash, no compatible biopsy, rapid resolution) push away from DRESS.
