Eosinophilic esophagitis histologic threshold

Checks the peak esophageal eosinophil count against the consensus EoE histologic threshold (≥15 eos/hpf) in the appropriate clinical context.

Evidence tier: Guideline-derived.

Formula / logic

EoE histologic threshold: peak ≥15 eosinophils per high-power field on esophageal biopsy. Diagnosis additionally requires symptoms of esophageal dysfunction and exclusion of non-EoE causes (per the AGREE consensus, the prior PPI-responsive EoE category was eliminated).

Interpretation

A peak ≥15 eos/hpf with compatible symptoms supports EoE. Treat as part of the full diagnostic context (endoscopy/EREFS, response to therapy).
