EGPA classification, 2022 ACR/EULAR criteria

Classifies eosinophilic granulomatosis with polyangiitis (EGPA) in a patient with established small/medium-vessel vasculitis.

Formula / logic

Sum: obstructive airway disease (+3) + nasal polyps (+3) + mononeuritis multiplex (+1) + blood eosinophils ≥1×10⁹/L (+5) + extravascular eosinophilic inflammation on biopsy (+2) + cANCA/anti-PR3 positivity (−3) + hematuria (−1). A score ≥6 classifies EGPA.

Interpretation

Sensitivity ~85%, specificity ~99% for EGPA vs other vasculitides in the derivation cohort. Negative items (cANCA/PR3, hematuria) favor an ANCA-associated GPA/MPA phenotype.
