Asthma Control Test (ACT)

Recall (4-week) asthma control, five items each 1-5 (range 5-25); ≥20 = well-controlled.

Evidence tier: Validated instrument.

Formula / logic

Sum of five items, each 1-5 (range 5-25). ≥20 = well-controlled; 16-19 = not well-controlled; ≤15 = poorly controlled. A change of ≥3 points is the minimal important difference.

Interpretation

Validated for patients ≥12 years. A separate Childhood ACT (c-ACT) exists for ages 4-11.
